2019-2020 SCHOOL YEAR

Temporary Student Service Crisis Case Manager
DISTRICT-WIDE


Job Goal: The Crisis Case Manager will provide immediate support, supply and immediate assistance as needed to students who are undergoing a clinical/health crisis. To coordinate the care given to students in crisis and to manage the student’s pathway during the episode of care. The Crisis Case Manager is also responsible to oversee the implementation of a follow up plan with support team at the student’s school and outside services to ensure the student is adequately supported and monitored for as long as is needed.

The Crisis Case Manager will also provide short-term assistance to family members to cope with the trauma associated with the student’s distresses. The will provide crisis support at the hospital or location of the family in collaboration with the school and other city agencies.

Responsibilities:

• Engage individuals and families as partners in services and develop positive, hopeful, and trusting relationships.

• Participate as an active member of the service provider team.

• Participate in the development of re-entry plans

• Mediate between students/families and other professionals directing services and care

• Exercise discretion and independent judgement when determining a course of action. Maintain the ability to compare and evaluate possible courses of conduct and actions, in consultations with Director of Student Services

• Build student relationships, as well as business relationships and strategic partnerships with other agencies and external resources

• Mediate between students/families and other professional services to facilitate care plans, strengths assessments and external resources

• Work with courts, social services, professional, doctors and education systems on behalf of the individual student/family to facilitate goals and objectives of the student, care team and organization

• Advocates for resources on behalf of the student and family with state, government and private agencies

• Recognize and demonstrate sensitivity to cultural and ethnic differences

• Demonstrate exceptional interpersonal skills and the ability to work with persons with severe and persistent mental illness and/or serious emotional disturbance, and the ability to react effectively in a wide variety of human service situations

• Regularly travel and transport students and/or families in and around the region, and provide direct care and support as needed.

• Be available to on an on-call basis

• This is a full time position and may include evening and weekend work

• Performs other tasks as assigned by the Director of Student Services

Qualifications:

• Possession of a Master’s Degree; in human services field, with emphasis preferably in social work, psychology, rehabilitation or related field.

• Valid driver’s license with motor vehicle insurance.

• Familiarity with mental health theory, emerging promising practices, and Evidence based Practice.

• Knowledge of trends and issues, laws and regulations related to the development and delivery of case management services helpful.

• Demonstrate interpersonal and communication skills.

• Demonstrate time management and organizational skills. The ability to manage priorities under changing circumstances and timeframes.
