2019-2020 SCHOOL YEAR

ANTICIPATED
Special Education Transition Specialist
DISTRICT WIDE

Job Goal:
To assist students to successfully transition out of Somerville High School, Full Circle and Out of District Placement.

Qualifications:
• Appropriate Department of Elementary and Secondary Education Educator's Licensure is required. (Moderate Disabilities 5-12, Counseling and SEI Endorsement required or ESL or ELL).
• Transition Specialist Endorsement preferred.
• Experience with college and career bound special education students.
• Ability to outreach and form long-standing relationships with colleges, community partnerships, and social service agencies.

Responsibilities:
• Coordinate, provide support and implement community based instruction, travel training and job placement.
• Develop employment, education, or internship opportunities for students with disabilities
• Act as lead in creation and implementation of college partnerships for current students.
• Participate in the College and Career Readiness team at Somerville High School.
• Design and teach 1-2 classes per semester such as Transition Prep and/or College Seminar.
• Provide executive functioning coaching, as needed, to students enrolled in college courses.
• Act as liaison with the Career and Technical Education (CTE) program at Somerville High School.
• Instruct students in the educational domains necessary for transitioning into adulthood- life skills, community and domestic skills, money management, vocational skills, etc. Provide academic instruction in class and small group settings to students.
• Consult and collaborate with staff around learning styles, curricular modifications, environmental accommodations, and alternative assessment.
• Develop a monitoring/record system for student’s job, community, and post-secondary educational experiences.
• Provide consultation to families about obtaining accommodations in college, technical/trade school, or employment.
• Involve parents in all areas of planning for their child’s education and assisting parents in accessing community resources to enrich their child’s educational program and transitional services.
• Manage caseload of students as needed, including: development of IEPs, writing progress reports, conducting educational assessments.
• Conducting transition assessments for students, as needed, throughout the district including assessment, report writing, and presentation at IEP meetings.
• Provide professional development, as needed, to students, families, and staff.
• Work collaboratively with Massachusetts Rehabilitation Commission (MRC), Departmento of Developmental Services (DDS), Department of Mental Health (DMH), MACEI, and other state and community agencies that support students as they transition out of high school.
• Develop and oversee the implementation of student’s individual schedules that may include employment, internships, community based activities, etc.
• Develop and implement person-centered plans for applicable students.
• Other related duties, as needed.
