SomerBaby Home Visitor

Job Goal: The Prenatal - Early Childhood Services and Supports Unit of the Somerville Family Learning Center works to develop & implement a comprehensive, research-based strategy for prenatal to early childhood supports, ensuring alignment & coordination with early childhood through K priorities, putting children & families on a path toward school success.

The SomerBaby Home Visitor offers a visit to all parents/guardians with newborns and provides an entry point for all children into Somerville’s continuum of services. The Home Visitor offers one-on-one support to parents, assesses mother and newborn health and well-being and provides education, support, and referrals to services as needed.

Preferred Qualifications:
• Experience with home visiting
• Bilingual English/Spanish and/or English/Portuguese required (written and spoken)
• Deep knowledge of family engagement, new parent support, and whole child approaches to early childhood development
• Excellent listening and communication skills
• Strong time management skills; organized; flexible thinker
• Solid understanding of challenges in a diverse, urban community
• Familiarity with issues unique to families with children with special needs
• Ability to work both as part of a team and in an individual setting
• A valid Massachusetts driver’s license with access to a car is required for anyone applying for this position
• Bachelor’s Degree preferred
• Flexible schedule preferred
• Coordinate closely with SPS Early Childhood, SPS Communications, Somerville HHS.
• Ability to work occasional evenings
• Record of collaboration with staff in a variety of roles
• Bilingual/multilingual and ability to communicate with diverse students and families

• Conduct outreach to all eligible families (Somerville residents, pregnant or with a newborn) and possible referral sources to identify newborns/infants; work to ensure that we keep pace with annual scaling up of goals (i.e. first goal is to reach 450 families and begin adding in follow up visits)
• Provide one-on-one support to parents through a welcome baby home visit
• Provide information and referral to families and document such referrals
• Offer welcome baby visits in English/Spanish and/or English/Portuguese. Provide translation or interpretation as needed.
• Coordinate with colleagues for other language capacity as needed.
• Complete intake forms and provide follow up (phone) survey
• Assess family strengths and needs and share information about community resources
• Collaborate with early childhood, early education and health community partners
• Conduct Ages and Stages developmental screening
• Review contents of materials routinely to ensure available inventory and to assess any changing needs
• Support emerging single point of entry system to assist families
• Help refine and evaluate the goals, objectives, and measurable outcomes of this project
• Provide feedback and documentation of the work, including on tools/templates/ activities/materials and on positive outcomes and areas for improvement
• Work collaboratively with other stakeholders (Schools, City, Community) to review effective services delivery
• Attend professional development trainings as needed to keep skills updated and acquire new relevant knowledge
• Execute all data reporting requirements
• Other tasks as assigned

Terms of Employment:
• This is a grant funded position
• 12-month work year
