Job Description
Case Manager for Special Education and 504 students / provide direct services as needed / obtain present levels and baselines/ review and print monthly online school snapshot of progress for reach student / follow up with providers to confirm that IEP attendance, reports and services are being completed / hold monthly scheduled phone conferences with Special Education families / turn in notes from emails and calls at the end of the month / be available for Special Education families on an as-needed basis via phone and on-line / ensure all documents are submitted prior to IEP meetings / hold transition meetings / monitor confidential tracking database for returned IEPs / work closely with Special Education Coordinator and/or Classroom Teachers / attend all special education meetings / develop lesson plans and instructional materials and provides individualized and small group instruction / translates lesson plans into learning experiences / establishes and maintains standards of student behavior / maintains professional competence through in-service education and activities / identifies student needs, communicates and cooperates with other professional staff members. Perform duties of equal or lesser value related to Pre-K Special Education teaching.
