**This position is for an on-call substitute nurse (LPN/RN)**

 

TITLE: District Substitute LPN/RN

 

CLASSIFICATION: Classified – Hourly

 

REPORTS TO: School Principal

 

SUMMARY: Under the direction of the School Principal, the Licensed Practical Nurse will provide health care services and help to minimize absences due to illness that will promote the fullest educational opportunity for students.

 

PERFORMANCE RESPONSIBILITIES:

  1. Identify, assess and evaluate the physical well-being of students and be aware of any of their mental and emotional problems.
  2. Maintain current cumulative health records for every student and protect the confidentiality of that information.
  3. Assess the areas of vision, hearing, dental health, obvious bone structure, deviations, skin conditions, genetic maladies, growth and development.
  4. Maintain current emergency card information.
  5. Provide accurate log of daily illness, accidents, treatment plan & parent communication.
  6. Assume the responsibility for emergency care for students who are ill or injured.
  7. Provide administration with injury or emergency reports for both student and school staff, and follow district procedures in contacting outside agencies when necessary.
  8. Maintain an accurate inventory & record of all controlled substance received with consent to administer as directed by the ordering Physician.
  9. Provide transportation to home, for students who are injured or become ill at school when necessary.
  10. Analyze and evaluate student immunization records and past medical histories in order to communicate need for      prevention and control of communicable disease and relate this information      to parents.
  11. Arrange and conduct hearings with parents who fail to comply with immunization policy of school district.
  12. Communicate to parents the procedures necessary to obtain homebound instruction for students absent with prolonged illnesses.
  13. Work with community agencies by providing data helpful in preventing or controlling communicable disease.
  14. Provide information on the modes of transmission of communicable disease to student, faculty and parents, and when necessary.
  15. Contribute to the decision-making process for placement of youngsters in special education classes by providing detailed medical and developmental histories.
  16. Communicate with parents relative to illness, injury, correction of health problems and the use of community resources.
  17. Inform the classroom teacher and other staff members of individual students with special health problems, recommending the adjustment of the educational program until problem can be corrected.
  18. Assist school personnel in understanding and accepting the student’s pressures from outside environment--parental, peer, and community.
  19. Counsel students in adjusting to their health status, correction of health problems and acceptance of their limitations.
  20. Serve as a health consultant and resource person in the school by demonstrating and giving instruction on health subjects.
  21. Provide faculty members with health information relating to specific screening programs, arranging for guest speakers and field trips.
  22. Supply faculty with current health and nutritional material including audio-visual aids and maintaining a library of health-related information for students and teachers.
  23. Serve as a liaison between parent, school and community in health matters.
  24. Participate in monthly district meetings designed for the improvement of the total school health program.
  25. Work cooperatively with Child Protective Services and other governmental agencies concerning the welfare of students.
  26. Administer medication in accordance with Board adopted policy.
  27. Provide emergency services for illnesses and injuries occurring at school and enlist the services of paramedics in accordance with plans and procedures developed by the principal when needed.
  28. Observe students to detect health needs including impetigo, scabies, and lice.
  29. Order & maintain a proficient amount of supplies for health services.
  30. Maintain a clean health office including the cleanliness of the beds, sinks, and health equipment.
  31. Provide assistance to students with severe health problems by riding the handicapped bus in the event of emergencies.
  32. Provide proficient services for special needs students including tracheotomy suctioning, tracheotomy care, G-tube feeding and care, cauterization, SVNTX via tracheotomy, when and if necessary as directed by ordering physician and scope of certification.
  33. Attend in-service training for continued professional development.
  34. Perform other job-related duties as assigned or required in the scope of certification.

EDUCATION:

  •  High school diploma or equivalency

LICENSES OR CERTIFICATIONS:

  • Current Arizona LPN license OR proof of candidate registration in an approved LPN education program.  (Candidate must provide documentation to HR every semester verifying progress towards meeting the LPN certification process). 
  • Arizona driver’s license and good driving record
  • Negative TB clearance within last 12 months

EXPERIENCE:

  • Previous experience in health care preferred

SKILLS/KNOWLEDGE/ABILITIES:

  • Bilingual English and Spanish, preferred
  • Knowledge and skills in providing nursing services
  • English proficiency in reading, writing, and speaking
  • Strong communication and interpersonal skills with demonstrated ability to work effectively with students, teachers, parents, and administrators of the targeted community
  • Able to work with detail and maintain accurate records
  • Good emotional and physical health
  • Able to bend, lift 40 pounds, and stand and walk for extended periods of time

WORKING CONDITIONS:

  • The working environment is a typical school nursing office.
