We are seeking a dynamic individual to lead our Social Services Department. As a Clinical Department Head, the Director of Social Services maintains census, facilitates admissions and tours for all prospective families. Responsibilities include but are not limited to :

•Facilitate tours for all prospective families
•Communication with families, ensuring an efficient admission process
•Building and maintaining facility occupancy through Admission and/or Discharge Planning for Post-Hospitalization, Short-Term and Long-Term admissions and Special Ed. School Programs
•Acquiring and fostering referral sources through use of NEPC marketing materials and presentations
•Interactions with hospitals, community pediatric liaisons and school districts specific to our population
•Assist in Guardianship acquisition

  • Job Requirements

    • •Excellent communication skills are essential in order to understand and coordinate care with the educational, nursing and rehabilitation teams
      •knowledge of ACO ( Accountable Care Organizations)
      •Experience working with government agencies
      •Desire to be in an intricate role that promotes and fulfils the growth of NEPC
      •Marketing and Admissions experience is advantageous for this position
      •Preferred candidate will have a Master’s Degree and current license in Social Work with 3 years’ experience in either pediatrics and/or the nursing home setting
