Job Description

Carry out the agency mission; supporting people to live fully, which includes the social, spiritual, cultural, recreational and personal growth and planning aspects of their lives.
Maintain the site and monitor individuals during the night as required. Continue professional development.
Location: North Grafton, MA
Schedule: Friday-Monday 10:30pm-8:30am

Maintain residence in a neat, clean and orderly fashion
Assist individuals in their morning routines, ensuring that they are appropriately prepared for the routines of the day
Provide awake coverage when required to assist individuals in evacuating during fire drills or in any emergency situation
Assist people with medications and health care needs, monitor and communicate health care issues
Physically assist people during emergency procedures or as needed for required care
Participate in the development of Individual Service Plans and implement ISP objectives consistently
Ensure that people’s human rights, privacy and confidentiality are respected
Ensure that people’s transportation needs are met
Role model appropriate behaviors, dress and social norms
Maintain communication with other staff through the use of the daily log, incident reports, etc.
Participate in all required trainings, staff meetings and supervision
Complete required trainings and maintain certification
Comply with all applicable policies, procedures, guidelines and regulations
Perform other reasonably related duties as assigned

Minimum Qualifications:
High school diploma or equivalency
Valid driver’s license (if required by site needs) and acceptable driving status, as determined by HMEA insurance company
Arrival at work on time, ready to work
Ability to work as individual and as part of a team in a reliable and responsible manner
